The Functional Wellness Assessment gathers information about your current symptoms, body signals, sensations, daily experiences, relevant health history, lifestyle factors and personal goals. This information helps create a broader picture of how your body responds, functions, adapts and recovers over time.

Please allow approximately 30 minutes to complete the assessment. There are no right or wrong answers. Simply provide the information that best reflects your current experiences, observations and goals.

Information gathered through the Functional Wellness Assessment is used to develop your Personalized Educational Summary (PES). The PES organizes your reported experiences into a functional overview designed to support observation, pattern recognition and individualized learning.

What Happens Next?

Once your PES has been completed, you will be invited to schedule a PES Review Session. During this session, we will review your summary together, discuss what stands out within your observations and explore potential next steps.

The fee for the Functional Wellness Assessment, Personalized Educational Summary (PES) and PES Review Session is $99. An invoice will be issued once your PES has been completed. Payment is due before the scheduled PES Review Session.

The assessment is educational in nature and is designed to support observation, pattern recognition, individualized learning and informed participation in healthcare and self-care.
