What is an Individual Support Plan (ISP)
What Is an ISP?
An ISP, or Individual Support Plan, is a personalized educational support document developed through the Somatic Self-Care System.
The ISP is designed to help organize awareness around functional patterns that may be influencing how a person feels throughout daily life.
Each ISP is developed individually using onboarding information, client-reported experiences and functional observations related to stress adaptations, breathing habits, recovery responses, movement patterns, tension buildup, daily demands and symptom timing.
The purpose is to support self-observation, recovery awareness and practical self-care adjustments that may help improve overall daily function and recovery.
What an ISP May Include
An Individual Support Plan may include:
- Functional observations related to stress and recovery patterns
- Breathing and tension awareness practices
- Supportive lifestyle and self-care considerations
- Recovery-oriented pacing and rest awareness
- Educational reflections related to tension and pressure patterns and daily function
- Suggestions intended to support awareness rather than symptom chasing
Each plan is supportive, educational and individualized rather than standardized or symptom-based.

Instead of focusing on diagnosis or symptom fixing, the goal is to support greater awareness of how the body may respond to , stress, pacing, breathing patterns and recovery demands over time.
Many people spend years searching for what is “wrong” without first learning how to observe what their body may already be communicating functionally.
