Functional Care and Wellness Support

The Somatic Self-Care System is an ongoing, self-care and recovery system designed to help individuals continue recognizing and organizing the body patterns and signals identified during their Functional Wellness Assessment . Rather than focusing on isolated symptoms, this support system helps organize how accumulated stress and ongoing demands may be influencing physical responses, symptom timing, tension and recovery patterns, adjustment patterns and overall function. Support focuses on understanding how patterns may develop, repeat, adapt and shift over time in response to recovery habits, movement, tension levels, breathing mechanics and environmental demands. This system exists alongside traditional healthcare — not to replace it but to support what happens between and beyond it, where many day-to-day patterns actually occur. What “Somatic” Means ...

What is an Individual Support Plan (ISP)

What Is an ISP?

An ISP, or Individual Support Plan, is a personalized educational support document developed through the Somatic Self-Care System.

The ISP is designed to help organize awareness around functional patterns that may be influencing how a person feels throughout daily life.

Each ISP is developed individually using onboarding information, client-reported experiences and functional observations related to stress adaptations, breathing habits, recovery responses, movement patterns, tension buildup, daily demands and symptom timing.

The purpose is to support self-observation, recovery awareness and practical self-care adjustments that may help improve overall daily function and recovery.

What an ISP May Include

An Individual Support Plan may include:

  • Functional observations related to stress and recovery patterns
  • Breathing and tension awareness practices
  • Supportive lifestyle and self-care considerations
  • Recovery-oriented pacing and rest awareness
  • Educational reflections related to tension and pressure patterns and daily function
  • Suggestions intended to support awareness rather than symptom chasing

Each plan is supportive, educational and individualized rather than standardized or symptom-based.

๐Ÿ›‘ Example Only

The sample ISP shown above is intended only to demonstrate the general structure and educational approach used within an Individual Support Plan.

It is not designed to be copied or used as a personal recovery plan. Every ISP is developed uniquely according to the individual’s own functional patterns, observations, stress responses and recovery needs.

The purpose of the example is to help visitors better understand how supportive guidance and functional awareness may be organized within the Somatic Self-Care System.

Functional Reflection

This example ISP demonstrates how stress load, breathing habits, tension responses and recovery capacity may interact throughout daily life.

Instead of focusing on diagnosis or symptom fixing, the goal is to support greater awareness of how the body may respond to , stress, pacing, breathing patterns and recovery demands over time.

Many people spend years searching for what is “wrong” without first learning how to observe what their body may already be communicating functionally.

Important Note

Individual Support Plans are educational and observational in nature and are not intended to diagnose, treat or replace medical care. This work functions alongside healthcare and focuses on supportive awareness, functional observations and recovery-oriented self-care practices.
